Allow model specific multi-layout configuration.

Previously it wasn't possible to customize 2nd, 3rd and 4th layout
based on model. Also rename the configuration files and scripts to
reflect added model column.

Introduce 'cz(dvorak-ucw)' layout.

It is a US Dvorak layout with the CZ accented characters accessible using
AltGr under the appropriate keys (eg. AltGr+a => aacute, etc.).

As an interim measure, replace AM_INIT_AUTOMAKE([dist-bzip2]) with
AM_INIT_AUTOMAKE([foreign dist-bzip2]). This will prevent the generation
of the INSTALL file. It is also part of the 24206 solution.

Minor tweaks have been made in comparison to code that was unjustifiably and abusively reverted by Sergey in February. In particular, crh.po is not checked in this time, because it was one of Sergey's reasons for reverting, so until it's pulled from TP, something like `cp po/tr.po po/crh.po` is required prior to building from source.
